What type of implants and how are they inserted?
There are many choices for implants and these choices need to be individualised to suit you. In general, it comes down to 4 main choices:
Do you want saline or silicone gel?
Saline is just salt water and of no consequence if it leaks. However, it tends to be firmer and doesn’t feel quite as natural as Silicone gel.
Silicone in the gel form doesn’t leak – it just sits there if the implant is cut. It is NOT the old silicon liquid that everyone was worried about.
Implants behind or in front of your chest muscle?
Factors influencing this decision are the thickness of the skin over your breast (and therefore over the implant), how big you want the implant and how active you are etc.
Round or anatomic (teardrop) implants?
If you have a nice shape but are just small, then a round implant is best. They also give the least trouble. If however you have had some sag and want your breasts to sit up a bit, then you may be looking at a teardrop shaped (anatomic) implant.
Placement via your armpit, under your breast crease or around your nipple?
Most of the time the implants insert best through an under the breast incision. There is less risk of nerve damage and they tend to sit better.
The scar hides nicely in the under crease of your breast. Sometimes via the armpit is OK – especially if you have small upturned breasts and no real crease to hide the scar. Through the nipple area is best avoided because the scar can be a problem and there is a significant risk of losing nipple sensation. It could be considered for saline implants that go in deflated and are then filled up once they are in place. You also need to be aware of general problems for all implants like bleeding, scar formation around the implant and infection.
It comes down to what you are trying to achieve and what looks natural and which is the safest for you personally.
